64 hours per two-week pay period. Day shift.
Coordinates team-based care to provide
health services to individuals, through effective partnerships with patients,
their caregivers/families, community resources, and their physician. Facilitates a “shared goal model” within and
across settings to achieve coordinated high-quality care that is patient- and
family-centered.
Current
license in the State of Minnesota as a Registered Nurse. Previous experience in
caring for chronic disease patients required. Minimum of two years in a
clinical setting, outpatient setting, or care coordination experience preferred.
Successful completion of the pre-employment health screening process required. Adequate
computer/keyboarding skills required.
$28.05 an hour.
Wage displayed is hourly minimum. Experience is taken into
consideration.
