Patient Navigator-Oral Medication

Description: 

The Patient Navigator will guide patients through the care continuum, establishing contact with newly diagnosed and recurrent cancer patients/family/caregivers. The Patient Navigator is responsible and accountable for the management of resources through interdisciplinary collaboration to achieve optimal patient outcomes for oncology patients. The Patient Navigator ensures the patient referral process and transition into specialty services is timely and efficient, anticipates patient / family/ caregiver needs throughout the continuum of care, communicates and facilitates care with multidisciplinary team members and community resources. This team provides the patients/families/ caregivers with clinical expertise, access to clinical trials, access to education and resources within their local community. The Patient Navigator will work closely with the other team members to identify and address community needs for cancer screening services and education related to cancer prevention and early detection. The Patient Navigator will be responsible for ongoing evaluation and quality improvement of the Navigator Program, provide supporting data for quality initiatives to maintain and improve operating processes. The ideal candidate requires excellent organizational skills, computer proficiency, is self-directed and able to organize and manage multiple projects simultaneously, possesses an extensive understanding of oncology patient care, excellent verbal and written communication skills, strong problem solving and advocacy skills, is articulate, compassionate and patient, enjoys working closely with patients, family, caregivers in identification of potential crisis situations and able to initiate appropriate action.

Requires a registered nurse with current licensure in Minnesota. A minimum of three years current medical nursing experience required. Oncology certification required within twelve months of hire. Must possess knowledge and ability to competently operate a computer and essential programs. This position requires current Healthcare Provider level CPR certification upon hire or must be obtained within 6 months.
