Description: 

The Community Health Initiative Project Coordinator is responsible for the day to day coordination of the community health initiatives, in order to deliver services that improve access, health and lower costs of a given population. This includes coordinating planning, implementation, evaluation, and maintenance of programs, facilitating systems change, multiple projects, grants, and partnerships within the overall community health and wellness strategy.  This includes coordination of services between Integrated Health Partnership, Medical Home program and the Accountable Communities for Health Partnership initially.  A strong working relationship between the program sponsors of these services will form the backbone for future community based health initiatives.  This position supports partnership and program implementation in a variety of settings including health care organizations, schools, business, government and nonprofit agencies.

Bachelor of Science or Bachelor of Arts in health related field with demonstrated project planning and management experience or Bachelor of Science in project management with demonstrated interest and experience in health care. At least two years of experience in project management or health promotion and/or community relations required.  May require evening or weekend work to attend meetings, or other events, as well as limited travel.  Valid driver’s license in good standing required. Excellent interpersonal, written and verbal communications skills. Proven ability to work effectively with teams, internally and throughout the community, with a high degree of professionalism, relationship-building, and collaboration. Understand the importance of local non-profit health care to the many different community stakeholders, and ability to connect hospital and clinic services to best benefit the organization and community stakeholders. Superior organizational skills with ability to prioritize, multi-task and meet deadlines. Demonstrated project management skills. Leadership skills to hire, terminate, and allocate resources to achieve stated goals. Ability to make independent decisions and adapt rapidly to change. Strong computer skills to support program development and reporting (Microsoft office including experience with Excel data management and PowerPoint presentations). Flexible, positive, poised with a desire to succeed. Cultural competency & a comprehensive understanding of issues of diversity.
