The Community Relations Coordinator develops relationships and provides education to staff, the community and to referral sources about the services Acute Rehab offers in order to increase our market share. The Community Relations Coordinator assists with referral network development, marketing, communication and promotion of Acute Rehab Services. This person will assist with FIM/CMG program development to ensure accurate capture of scores and maximize reimbursement. Excellent interpersonal skills and communication skills are essential for this position. This person will conduct on-site pre-admission assessments as well.
Graduate of School of Nursing with current licensure in MN or PT/OT/ST with current licensure in MN. Preference for this position is that candidate has 5 years experience in Acute Rehab or has certification as a Rehab Nurse. This position requires current Healthcare Provider level CPR certification upon hire or must be obtained within 6 months. Requires valid driver’s license and good driving record. Able to sit and drive for long periods of time. Able to drive a car independently as this position requires a great deal of travel. Requires strong communication skills. This person must have good problem solving skills, be self driven, have initiative and exercise good judgment in referring the individuals they see.
